Required Forms

The following forms must be on file each calendar year in order to be able to participate in Therapeutic Recreation programs:

• Participant Information Form
• Liability Release
• Assessment

Registration Forms

Fall Registration begins August 29th and is on a first come, first served basis. Register early – programs fill quickly!

• Little Rockers Kids Marathon Walking Program – School/ Class room Participation (Teachers please note, you are required to submit the class sign –up form to register your group AND individual registration forms – both are included in this link)
• Little Rockers Kids Marathon Walking Program – Non-School Participants
